Hospital Discharge Service

If you are over 60 and have just been discharged from hospital or have come home after an unfortunate visit to the Accident & Emergency Unit, you may be a little uncertain as to how you are going to cope in the few weeks ahead.

This is where our friendly and experienced Settlement aides can be there for you. They are on hand to help, to listen and provide a friendly face, Whether you live alone or with a partner/ family but they work or live away and are not on hand to help—we can.

There is no charge for this service; however it is short term ie on average 3-4 visits.
